An Online Evaluation & Reporting System for Biokineticists

Components

Practice Admin

The application allocates one Super Administrator username and password. The Super Administrator will be responsible to maintain the whole application, including maintaining the practice information, users, accounts and access to the evaluation forms and reports globally. Practise details are first captured before additional users can be set-up, which will be directly linked to a practice or practices.

User Admin

A user will have access only to specific information allowed to them, depending on the security level allocated to the user. These security levels consist of: Biokineticist and Administration. Only Administrators can perform maintenance on users.

Patients

A patient profile is set-up for every patient. Patient information will only be accessible by the Biokineticist that treats the specific patient to secure a confidential “doctor patient” relationship. If a Biokineticist can not attend a meeting with a specific client, the client information can temporarily be transferred to another user.

Forms

The evaluation forms and reports form a part of the main functionality of the application. The forms can logically be separated into three main sections:

General Information

General Injury Information: When a new patient is being evaluated, their injury history and other evaluations are recorded with this form.

Evaluation Forms

  • Wellness Assessment: The wellness assessment form is used for assessing a patients physical and fitness conditions.
  • Shoulder Evaluation: Form for recording the evaluation of the shoulder(s).
  • Knee Evaluation: Form for recording the evaluation of the knee(s).
  • Spinal Evaluation: Form for recording the evaluation of the spinal condition.
  • Hip/Thigh Evaluation: Form for recording the evaluation of the hip(s) and thigh(s).
  • Elbow & Hand Conditions Evaluation: Form for recording the evaluation of the elbow(s) and hand(s).
  • Neurological Conditions: Form for recording the evaluation of the neurological condition. This form contains a shared neurological evaluation and functional tests sections, but these sections contain a host of tests which differ from the other forms.
  • Lower Leg, Ankle & Foot Conditions Evaluation: Form for recording the evaluation of the lower leg(s), ankle(s) and foot conditions.

BIO Treatments

  • Home Program: The home program form is completed by the biokeneticist. It is intended to give a patient a home training program to follow when not being treated.
  • Rehabilitation Program: The rehabilitation form is completed by the biokeneticist. It follows the same structure as the home program mentioned earlier.
  • SOAP Form: The soap form is a free-format style form which the biokeneticist use in recording progress in the treatment of a patient.

After a form is completed, the separate data on the form is stored in their respective formats. This data can then be retrieved as needed for viewing or printing. Forms can be viewed on-line, they can be printed or a PDF document can be created and saved on the users’ local computer for later off-line viewing. The same information can be extracted from the database and displayed in a report to be sent to a referring doctor as feedback.

Reports

The application allows users to create reports.

  • Bio-Rehab Report: Certain areas of completed evaluations will be extracted from the database and presented in a report.
  • Isokinetic Report: This report is generated manually using the information gathered using any of the electronic assessment machines.
  • Practice Account: This account form relates to a specific date’s activities for a certain practice.
  • Patient Account: This account form relates to a specific patient’s account with a practice or Biokineticist for a specific period.
